Hospitals Sign On to Reduce Avoidable Readmissions

The following hospitals have signed on to support the RARE Campaign and reduce overall readmissions by 20% from their 2009 baseline—as measured by Minnesota Hospital Association Potentially Preventable Readmissions (PPR) data—by December 31, 2012. The Operating Partners started recruiting hospitals mid-July 2011. New hospitals will be added as they join.

Their commitment underscores the leadership that's needed to significantly reduce avoidable readmissions—an issue that requires collaboration among community stakeholders. Reaching our goals will enable us to achieve the Triple Aim of improving population health, the patient care experience and the affordability of care in this area.
